SMHA Affiliation Request Form

This form is to be used when SMHA teams are requesting to affiliate a SMHA player from another SMHA team. This form should only be used by the SMHA Head Coach or Managers.

When selecting a player, make sure you reference the SMHA Affiliation Matrix to ensure you are selecting a player from an appropriate team.
