SMHA Multiplayer Family Rebate Application

FAMILIES WITH 3 OR MORE REGISTERED PLAYERS WILL BE ELIGIBLE FOR A MULTI-PLAYER DISCOUNT. THREE REGISTERED PLAYERS WILL BE ELIGIBLE FOR A 5% REBATE AND FOUR OR MORE REGISTERED PLAYERS WILL BE ELIGIBLE FOR A 10% REBATE (REGISTRATION FEES ONLY AND FUNDING AGENCY PAYMENTS ARE NOT INCLUDED).
APPLICATIONS MUST BE RECEIVED BY JANUARY 15th AND REBATES WILL BE PROCESSED NO LATER THAN JANUARY 31st.  NO LATE APPLICATIONS WILL BE PROCESSED.
